A Note On Public Assistance Applications

If you sustained damage from Hurricanes Gustav and Ike you must submit a Request for Public Assistance (RPA) for both storms. They are individually declared disasters and must have individual RPA's to be eligible for Public Assistance for the separate events. While the 14 Parishes (Acadia, Beauregard, Calcasieu, Cameron, Iberia, Jefferson, Jefferson Davis, Lafourche, Plaquemines, Sabine, St, Mary, Terrebonne, Vermillion and Vernon) may become eligible for additional categories of damage, they have already been declared for Category A and B and it is imperative that you submit an RPA for Hurricane Ike. Guidance for submitting the necessary form follows.

The Public Assistance Section of The Governor’s Office of Homeland Security and Emergency Preparedness would like to call your attention to the online availability allowing you to complete your Request for Public Assistance in accordance with 44 CFR § 206.202(c)

Current users must login to their account online and click on “Submit for a New Disaster” field on the summary page of their account.  Fill out all applicable fields of the RPA online and submit to the State when complete. For new applicants who do not currently have user access they can manually complete the form. The form can be located on the Louisianapa.com home page. Select the Resources tab and the Request for Public Assistance will be found under the forms section heading. This form can be printed, manually completed and faxed to 225-267-2832 with any attachments required.

In order to expedite processing of requests we ask that all applicable information is filled out and that phone numbers and e-mail addresses are complete and accurate. Private Non-Profit Organizations must attach copies of their Tax Exemption Certificate and Organization Charter or By-Laws with the request.  In addition if an organization is a school or educational facility, they should attach information on accreditation or certification with the request.

If you have any questions concerning filling out your Request for Public Assistance online please contact LaDonna Strauss at 225-339-6646 or via email at LStrauss@OHSEP.Louisiana.gov
